LHRH agonists (Goserelin/Zoladex, Leuprolide/Lupron, Triptorelin) work by suppressing pituitary LH and FSH secretion, thereby reducing testosterone (in men with prostate cancer) or oestrogen (in pre-menopausal women with breast cancer) to castrate levels. At our centre, these injections are administered monthly or three-monthly as depot preparations. In prostate cancer, LHRH agonists may be combined with anti-androgens (Bicalutamide, Enzalutamide, Abiraterone) for complete androgen blockade. In breast cancer, they are combined with Tamoxifen or aromatase inhibitors based on age and risk stratification. Monitoring includes PSA levels (prostate), hormone levels and bone density (DEXA scan).
