Febrile Neutropenia Management

Urgent assessment and antibiotic treatment for fever in chemotherapy patients with low white cell counts — a potentially life-threatening emergency.

Emergency Chemotherapy Neutropenia
Febrile neutropenia — defined as fever (temperature >38.5°C) in a patient with an absolute neutrophil count below 0.5×10⁹/L — is an oncological emergency requiring immediate medical attention. The risk of bacterial sepsis in this setting is very high and mortality can be significant without prompt intervention. At our centre, patients on chemotherapy are clearly counselled about this risk before treatment begins and are given emergency contact instructions. Assessment includes blood cultures, full blood count, CRP, renal and liver function, and a chest X-ray. Empirical broad-spectrum antibiotics are started immediately while awaiting culture results, and G-CSF support is considered. Risk stratification (MASCC score) guides the decision between outpatient and inpatient management.
