Blood transfusions — of red cells, platelets and fresh frozen plasma — are an important supportive measure in the management of cancer and haematological disorders. Patients with symptomatic anaemia (haemoglobin typically below 7–8 g/dL), severe thrombocytopenia with bleeding risk (platelet count below 10–20×10⁹/L), or coagulopathy may require transfusion support. At our centre, all transfusions are administered following current clinical guidelines and with appropriate cross-matching. We take a restrictive transfusion approach where clinically safe, as unnecessary transfusions carry risks (alloimmunisation, transfusion reactions, iron overload). Pre-transfusion testing, monitoring during infusion and post-transfusion assessment are all standard practice. Patients with haemoglobin disorders (thalassaemia, sickle cell) requiring regular transfusions are managed with iron chelation therapy to prevent transfusion-related iron overload.
